Information you provide
When you request an appointment, guide or response, forms may ask for your name, email, telephone number, mailing address and message. Providing information is voluntary, but the website operator needs enough information to respond. Your request is delivered to the website operator by email using SendGrid. The contact form does not maintain a separate archive of message contents in the website database. Authorized website service providers may process the request to operate and maintain these functions.
Use general contact forms for routine inquiries. Do not include Social Security numbers, payment-card details or detailed medical histories in a general contact form. Website forms are not emergency services; call 911 for an emergency.
How information is used
The website operator uses submitted information to respond to your request and help connect you with an appropriate local center. Contact and location details you provide may be shared with that center for the requested follow-up. Hosting, security and form services process technical information to deliver pages, prevent spam and abuse, troubleshoot errors and maintain the website. Technical information can include IP address, browser and device details, request time and the page requested. Requesting a response is separate from agreeing to optional website tracking or unrelated marketing.
Analytics and external content
We use Google Analytics and first-party directory statistics to understand visits to this U.S.-focused website. Analytics start automatically unless you have opted out or your browser sends a supported Global Privacy Control signal. Our cookie notice explains this use; selecting “Got it” only dismisses the notice. Advertising tags, advertising cookies, Google signals and advertising personalization are disabled by the website’s tracking configuration. The non-confidential treatment or content license applicable to public comments, unsolicited business ideas and creative suggestions does not apply to private patient inquiries or personal health information. Do not post personal or health information in publicly visible comments. General contact forms are for routine inquiries; use the local center’s designated intake process for detailed medical information. Submitting a form does not create a doctor-patient relationship.

No agency, partnership, joint venture, employer‑employee or franchiser‑franchisee relationship is intended or created between you and Company or its officers, directors, employees, agents under this Agreement or otherwise.

DCOA AI Assistant
The optional AI assistant answers general educational questions and helps find offices. It does not diagnose conditions, provide individualized treatment, schedule appointments, or replace a healthcare professional. Do not submit sensitive medical details, medical records, payment information, or personal identifiers.
When you send a message, your question and recent conversation context may be sent to OpenAI to generate a reply. We request that API responses not be stored as persistent response objects; OpenAI's own processing and retention policies also apply. Questions, replies, suggested offices, timestamps, and the page where a conversation started are saved privately for a 30-day review period so authorized DCOA administrators can review the assistant's responses. Chats are not public. A necessary session cookie maintains the conversation; starting a new chat does not delete the earlier log. Expired logs are removed automatically when the assistant or admin logs are next accessed; on an idle site, physical removal occurs at that next access.
A ZIP or postal code you provide is used to search the directory and may appear in the conversation log. Office suggestions show the nearest active offices with full websites; the full locator also includes other listings. Distances are approximate. Chat is not monitored for emergencies; call 911 or seek emergency care when needed.
Contact Messages
When you submit the contact form, your name, email, optional phone and ZIP or postal code, chosen topic, and message are sent through Twilio SendGrid to the DCOA team for responding to your inquiry. Please do not include medical records or sensitive health details. The website stores only the latest sending status, not a separate archive of message contents; copies remain with the email provider and in the recipient’s mailbox under their retention practices. A necessary session cookie protects the form and helps prevent duplicate submissions. Short-lived, hashed network identifiers are used to limit spam. When reCAPTCHA v3 is enabled, submitting this form loads Google’s bot-protection service, which processes browser and device signals and may set a necessary security cookie. We verify its security token before sending your message; our verification request contains the token and secret, not your form fields. Google’s Privacy Policy and Terms of Service apply. Contact form values are not included in the website’s conversion event.
Website and Location Directory Analytics
When Google Tag Manager is enabled, Google Analytics loads automatically to measure website visits, successful contact-form submissions, and event counts for locator searches, phone and website clicks, directions, and use of the chat assistant, unless you have opted out or your browser sends a supported Global Privacy Control signal. Google may receive device and browser information, page addresses, interactions and online identifiers, and may use analytics cookies. The website configuration restricts Tag Manager to analytics tags, keeps advertising storage and personalization disabled, and turns off Google signals. See how Google uses information from sites that use its services. These action events do not include chat messages, search terms, ZIP codes, office identifiers, phone numbers, email addresses, or form contents. Google Analytics still uses its standard browser and session information. Private management pages and chat logs do not include Tag Manager.
The location directory also measures visits to office pages and clicks on office phone, email and website links. These statistics start automatically unless you opt out or send Global Privacy Control. Selecting “Got it” acknowledges the cookie notice; it is not recorded as consent or a change to your analytics preference. Analytics changed from an affirmative opt-in to this notice-based approach on October 7, 2026 (UTC). Previously saved opt-outs remain effective.
When enabled, a random browser identifier is stored on your device. The directory records the office identifier, action type, date and time, and whether the action occurred in the locator or on an office page. The browser identifier is hashed on the server. These analytics do not include your name, email address, phone number, search text, or precise geographic coordinates. We check the browser’s user-agent information for identified automated traffic and check event timing for rapid repeats. Quality flags may be stored with events; full user-agent strings are not stored in these analytics. Event records are retained for up to 395 days.
You can change your preference using “Privacy Choices” in the notice or footer. Turning analytics off stops future directory measurement, removes the directory browser identifier and accessible Google Analytics cookies, and reloads the page if Google tags were already loaded. It does not retroactively delete previously collected information. Browsers that send a Global Privacy Control signal are treated as opting out and Google analytics tags are not loaded for them.
Your browser asks for permission before providing your location. Distances to nearby offices are calculated in your browser. To fill in your ZIP or postal code, your browser's coordinates are sent securely through this site's server to Google Maps. We do not store these coordinates or include them in directory analytics. Google handles this lookup under its own privacy policy. Manually entered city and postal-code searches are handled by this site's directory. Viewing the interactive map connects to OpenStreetMap's tile service; following directions, office website links, or email links opens the chosen external service. These services have their own privacy practices.
